A professional assessment of mobility, strength and the home environment, used to identify the right equipment, adaptations and exercises to keep someone safe and as independent as possible.
Our approach
Understanding what will help, before we start.
Where mobility, strength or the safety of the home is a concern, we can arrange for a qualified occupational therapist or physiotherapist to carry out an assessment. This looks at how someone is currently managing and what would help them move, function and stay safe more easily at home.
The findings are used to shape the care plan, and are shared with the person, their family and GP so that everyone is working from the same recommendations.
What we assess
A thorough look at mobility and the home
Assessments are tailored to the individual. Depending on the situation, they can cover:
- Home layout, mobility and fall risk
- Strength, balance and functional movement
- Equipment needs, such as rails, hoists and walking aids
- Home adaptations, such as ramps, stairlifts and bathing equipment
- Exercise programmes to support recovery
- Recommendations shared with GPs, discharge teams and family
